How to Prepare an Emergency Kit
In some emergencies, you may not have access to your regular dialysis treatments, or you may not be able to access the foods you usually eat. Take action now by preparing an emergency kit so you can still receive the treatment you need or lessen the impact of having to miss a dialysis session.
Emergency Kit Basics
An extra 3-day supply of all medicine (for kidney disease and any other conditions).
Necessary medical supplies and equipment, including any home dialysis supplies.
At least 3 days’ worth of food that fits the Emergency Eating Plan provided to you by your dietitian.
A Personal Information List with your ID, medication and allergy lists, insurance card, emergency and doctor contacts, dialysis treatment information and the DaVita Guest Services phone number 1-800-400-8331.

About DaVita Guest Services
We serve patients and support the community with:
- Emergency assistance. We support patients and DaVita teammates when a natural disaster or severe weather event strikes.
- Dialysis center placement. We arrange for permanent dialysis center placements due to relocation, hospital discharge or patient preference.
- Travel dialysis placement. We place DaVita patients nationwide (including Alaska, Hawaii and Puerto Rico) in both DaVita and non-DaVita centers.
